Arachnoid web is a rare disease entity that can cause progressive myelopathy and most often develops at the upper thoracic level. Its pathophysiology unclear, but may be associated with degeneration of septum posticum in dorsal subarachnoid space, which alters flow cerebrospinal fluid (CSF) subsequently leads to cord compression syringomyelia. It presents pain, paresthesia, extremity weakness. diagnosed by typical pattern displacement spinal cord, known as scalpel sign, intact ventral dura mater disturbed conserved CSF flow. should differentiated from other entities sharing feature such arachnoid cyst or herniation. The treatment for surgical resection. We report 66-year-old female who was dorsum T3 She had suffered weakness both legs 3 months. underwent laminectomy T4 opened. resected then improved.
